Tattooing Session

Whether you had double incision, periareolar, keyhole, or inverted-T surgery — with or without nipple grafts — this technique is fully adaptable to your anatomy and your vision.
Your Chest, Your Choice
Unlike traditional medical tattooing, gender-affirming areola work centers your autonomy. You decide everything:
- Size and shape — from anatomically proportional to intentionally asymmetric
- Color — matched to your natural skin tone or a preferred shade
- Style — hyperrealistic 3D nipples, subtle color wash, minimalist, or decorative/artistic
- Placement — mapped to your chest anatomy for the most natural result, or positioned exactly where you want them
Some clients want areolae that look indistinguishable from natural tissue. Others want something uniquely theirs. Both are equally valid, and we're here to execute your vision with the same precision and care.
Working With Post-Surgical Tissue
Top surgery creates unique tissue characteristics that require specialized expertise:
- Scar tissue integration — We tattoo over and around healed surgical scars, blending them into the design rather than fighting against them. Double incision scars, periareolar scars, and drain site marks can all be incorporated.
- Nipple graft enhancement — If you have nipple grafts that have lost pigment, flattened, or healed unevenly, we can restore color, add definition, and create a more natural appearance.
- Flat chest creation — If you opted for no nipple grafts or had free nipple grafts that didn't survive, we can create the complete illusion of a nipple-areola complex on flat skin using 3D shading techniques.
- Skin type adaptation — Post-surgical skin on testosterone may behave differently than it did pre-HRT. We adjust pigment selection, needle depth, and technique based on your individual skin characteristics.
Recovery & Aftercare
Healing from gender-affirming areola tattooing follows the same straightforward process as other paramedical tattoo procedures:
- Days 1–3: Mild tenderness, slight redness, and the area may feel warm. This is normal.
- Days 4–10: Light flaking and peeling as the surface layer heals. The color will appear lighter temporarily — this is expected.
- Weeks 4–6: Pigment stabilizes beneath the skin and the true color emerges.
Safety & Suitability
Who Is a Good Candidate for Areola Tattooing After Top Surgery
You’re generally a good candidate if:
- It has been at least 6 months since your most recent top surgery or revision
- Your incisions and nipple grafts (if present) are fully healed, closed, and stable
- Your scars are no longer open, weeping, or actively changing in a concerning way
- Your surgeon has provided a signed medical clearance confirming:
- Surgical sites are fully healed and stable
- There are no active complications (infection, wound breakdown, seroma, etc.)
- They approve cosmetic tattooing on the treated area
- You are able to follow aftercare instructions and attend touch‑up sessions if needed
Ideal Timing
- Minimum: 6 months after your final top surgery or most recent revision
- Ideal: 9–12 months post‑op for:
- More mature, settled scars
- More predictable pigment retention
- Less risk of tattooing into unstable tissue
- If you’ve had revision surgery, the waiting period restarts from the most recent procedure date.
- Nipple grafts should be:
- Fully attached and stable
- Free of scabs, open areas, or drainage
- Cleared by your surgeon for tattooing
Clients on Testosterone (HRT)
Testosterone does not prevent you from safely receiving areola tattoos.
You can be a good candidate while on HRT if:
- Your hormone regimen is stable or any recent changes are known and discussed
- Your skin is healed from surgery and free of active chest‑area conditions
How we adapt care for clients on testosterone:
- Increased sebum (oil) production:
- We may adjust needle depth and pigment density to improve retention
- Texture changes and thicker skin:
- We account for this in technique and may recommend specific aftercare
- Increased chest hair:
- Hair in the area is normal; we may trim or shave the immediate tattoo zone
You do not need to stop or change your HRT for this procedure. During consultation, we’ll ask about:
- Your current HRT status
- Dosage and route (e.g., injections, gel, patch)
- Any recent changes in dose or medication
This helps us plan your session and healing expectations more accurately.
Who Should Wait or Get Medical Clearance First
You should delay booking and/or consult your doctor if any of the following apply:
1. Active Skin Issues on the Chest
- Eczema, psoriasis, dermatitis
- Any active rash, infection, or open area
- Fungal or bacterial skin infections
These conditions can:
- Increase risk of infection
- Affect pigment healing and retention
Action: See your primary care provider or dermatologist. Tattooing should only be done on calm, intact, non‑inflamed skin.
2. Keloid or Hypertrophic Scarring
- History of keloids (large, raised scars that extend beyond the original wound)
- History of hypertrophic scars (thick, raised scars within the wound line)
Tattooing into or near keloid‑prone areas can:
- Potentially worsen raised scarring
- Lead to unpredictable cosmetic results
Action: A dermatology or surgical evaluation is strongly recommended. In some cases, tattooing into keloid tissue may not be advisable.
3. Unhealed or Unstable Scars
Avoid tattooing if scars are:
- Still pink, bright red, or very dark from recent healing
- Raised, spreading, or thickening
- Itchy, painful, or changing in shape/texture
These are signs the scar is still maturing.
Action: Allow more time (often closer to 9–12 months post‑op) and obtain surgeon clearance before proceeding.
4. Pregnancy or Breastfeeding
For clients who may become pregnant:
- Pregnancy and breastfeeding are times when the body and immune system are changing
- There is limited safety data on elective tattooing in these periods
Action: Discuss timing with your OB‑GYN or primary care provider. Many clients choose to wait until after pregnancy and breastfeeding are complete.
5. Blood‑Thinning Medications & Supplements
Includes but is not limited to:
- Prescription anticoagulants (e.g., warfarin, apixaban, rivaroxaban, heparin)
- Daily aspirin or high‑dose NSAID use
- Supplements that increase bleeding risk, such as:
- Fish oil / omega‑3s
- High‑dose vitamin E
- Ginkgo, ginseng, garlic, or similar
These can:
- Increase bleeding during the procedure
- Affect pigment placement and healing
Action: Do not stop any prescribed medication without medical advice. Speak with your prescribing provider about whether tattooing is appropriate and if any adjustments are safe.
6. Immunosuppressive Medications or Conditions
Includes:
- Medications for autoimmune diseases (e.g., biologics, steroids, methotrexate)
- Post‑transplant immunosuppressants
- Immunosuppression after recent major surgery or chemotherapy
These can:
- Increase infection risk
- Slow healing
Action: Get explicit clearance from your treating physician before scheduling.
7. Recent Chest‑Area Procedures
Within the last 3 months, if you’ve had:
- Laser treatments
- Steroid injections into scars
- Microneedling, chemical peels, or other dermatologic procedures
The skin may be more fragile or reactive.
Action: Wait at least 3 months after these treatments and confirm with your provider that the skin is ready for tattooing.
Safety Standards You Can Expect
Every session is performed with clinical‑level safety protocols:
- Single‑use, sterile needles and cartridges only
- Hospital‑grade disinfection of all surfaces and equipment between clients
- Medical‑grade, hypoallergenic pigments that are MRI‑safe and biocompatible
- Full OSHA bloodborne pathogen compliance, including:
- Proper sharps disposal
- Gloves and barrier protection
- Written aftercare instructions at every session so you know how to care for your tattoo as it heals
